{"id":2304,"date":"2024-06-06T19:00:15","date_gmt":"2024-06-06T22:00:15","guid":{"rendered":"https:\/\/anibalsilveira.org\/?page_id=2304"},"modified":"2025-05-05T20:07:54","modified_gmt":"2025-05-05T23:07:54","slug":"introducao-a-psiquiatria-sociologica-2","status":"publish","type":"page","link":"https:\/\/anibalsilveira.org\/en\/introducao-a-psiquiatria-sociologica-2\/","title":{"rendered":"Introdu\u00e7\u00e3o \u00e0 Psiquiatria Sociol\u00f3gica\u00a0"},"content":{"rendered":"\n<p class=\"has-text-align-center\"><strong>Introdu\u00e7\u00e3o \u00e0 Psiquiatria Sociol\u00f3gica&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"has-text-align-right\"><strong><sub>Alexandre Valverde Marcondes de Moura<br>Flavio Vivaqcua<br>Francisco Drumond Marcondes de Moura<br>Paulo Palladini<br>Roberto Fasano Neto<\/sub><\/strong><\/p>\n\n\n\n<pre class=\"wp-block-preformatted\"><strong>Vers\u00e3o 2 (05\/05\/25): vers\u00e3o preliminar que na medida que for ampliada ser\u00e1 substitu\u00edda<\/strong><\/pre>\n\n\n\n<p><strong>Conceito<\/strong><\/p>\n\n\n\n<p>Em nosso modo de ver, denominamos Psiquiatria Sociol\u00f3gica a psiquiatria que, partindo de uma abordagem sociol\u00f3gica sobre a din\u00e2mica e a estrutura ps\u00edquica, em correla\u00e7\u00e3o com a atividade cerebral, estuda e pratica a participa\u00e7\u00e3o sociocultural na origem, na preven\u00e7\u00e3o e no tratamento do sofrimento ps\u00edquico, compreendido na sua singularidade, em qualquer uma de suas modalidades: quadro reativo atual, ocasional, permanente, diat\u00e9tico, revers\u00edvel ou progressivo.&nbsp;<\/p>\n\n\n\n<p>A teoria que adotamos e que norteia essa abordagem \u00e9 a Teoria Sociol\u00f3gica da Personalidade, assim caracterizada por An\u00edbal Silveira em 1977<sup data-fn=\"bba11fc5-49e5-469a-a9d6-157f0042934c\" class=\"fn\"><a href=\"#bba11fc5-49e5-469a-a9d6-157f0042934c\" id=\"bba11fc5-49e5-469a-a9d6-157f0042934c-link\">1<\/a><\/sup>. Dessa forma, a Psiquiatria Sociol\u00f3gica \u00e9 a psiquiatria reformulada, a partir da aprecia\u00e7\u00e3o do conjunto do conhecimento acumulado nesse campo, sob o prisma da Teoria Sociol\u00f3gica da Personalidade.<\/p>\n\n\n\n<p><strong>Princ\u00edpios:<\/strong><\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>O contexto social e hist\u00f3rico \u00e9 um fator patog\u00eanico relevante, no \u00e2mbito do que compreendemos e que est\u00e1 sendo comprovado, ampla e sistematicamente, por evid\u00eancias como \u201cprodu\u00e7\u00e3o social do sofrimento ps\u00edquico\u201d.&nbsp;<\/li>\n\n\n\n<li>Nesse sentido, consideramos consistente a afirma\u00e7\u00e3o de que o sofrimento ps\u00edquico, em todas as suas modalidades, tem a sua origem na intera\u00e7\u00e3o de determinadas din\u00e2micas sociais e culturais com as disposi\u00e7\u00f5es biol\u00f3gicas, fisiogen\u00e9ticas que constitui, no seu conjunto, a singularidade de uma determinada pessoa, de um determinado agente social<sup data-fn=\"c5e96b75-aeb9-45ca-848b-aae540238371\" class=\"fn\"><a href=\"#c5e96b75-aeb9-45ca-848b-aae540238371\" id=\"c5e96b75-aeb9-45ca-848b-aae540238371-link\">2<\/a><\/sup>.<\/li>\n\n\n\n<li>Subentendida dessa forma, essa abordagem n\u00e3o supera apenas a dicotomia mente\/corpo, mas estabelece a possibilidade de proposi\u00e7\u00e3o de uma teoria explicativa que aponta para a integra\u00e7\u00e3o mente\/corpo\/ambiente f\u00edsico e social.<\/li>\n\n\n\n<li>Aqui vale ressaltar que compreendemos a mente como \u201cmundo interno subjetivo\u201d e o corpo como \u201cmundo interno objetivo\u201d incluindo aqui, necessariamente, o c\u00e9rebro. Dessa forma, a atividade ps\u00edquica aparece como uma qualidade superior da rela\u00e7\u00e3o entre esses tr\u00eas mundos: o mundo interno subjetivo, o mundo interno objetivo e o mundo externo. Importante assinalar que reconhecemos o equ\u00edvoco de estabelecer uma vis\u00e3o reducionista da din\u00e2mica ps\u00edquica com a atividade cerebral.<\/li>\n\n\n\n<li>O contexto social e hist\u00f3rico deve ser compreendido nos seus diversos aspectos:<\/li>\n<\/ol>\n\n\n\n<ol class=\"wp-block-list\">\n<li>a diversidade cultural das variadas formas de organiza\u00e7\u00e3o das sociedades humanas consideradas no \u00e2mbito do seu tempo hist\u00f3rico;<\/li>\n\n\n\n<li>a diversidade racial que se desenvolve no bojo do processo evolutivo da esp\u00e9cie humana;<\/li>\n\n\n\n<li>a atual hegemonia de um modo de produ\u00e7\u00e3o que imp\u00f5e o trabalho na sua forma negativa, isto \u00e9, como nega\u00e7\u00e3o da criatividade e da plena express\u00e3o das potencialidades humanas, acarretando um empobrecimento do processo de mobiliza\u00e7\u00e3o dos recursos internos subjetivos com impacto sobre a sociabilidade;<\/li>\n\n\n\n<li>nesse mesmo sentido, como se d\u00e1 a inser\u00e7\u00e3o no mundo do trabalho ou n\u00e3o, acarretando com isso um vasto contigente de pessoas excluidas e, portanto, desvinculadas das condi\u00e7\u00f5es necess\u00e1rias para a manuten\u00e7\u00e3o b\u00e1sica da sua sobreviv\u00eancia.\u00a0<\/li>\n<\/ol>\n\n\n\n<p>Em um instigante artigo<sup data-fn=\"6ad54778-38a2-484d-8470-817d777711b7\" class=\"fn\"><a href=\"#6ad54778-38a2-484d-8470-817d777711b7\" id=\"6ad54778-38a2-484d-8470-817d777711b7-link\">3<\/a><\/sup>, Diane e Nikolas Rose fazem uma abrangente revis\u00e3o bibliogr\u00e1fica sobre uma s\u00e9rie de abordagens que prop\u00f5em novos modelos para teorias e pr\u00e1ticas psiqui\u00e1tricas, avaliando quatro aspectos:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>como incorporam dimens\u00f5es &#8216;sociais&#8217;,&nbsp;<\/li>\n\n\n\n<li>como envolvem &#8216;comunidades&#8217; no tratamento,<\/li>\n\n\n\n<li>como engajam os usu\u00e1rios dos servi\u00e7os de sa\u00fade mental,<\/li>\n\n\n\n<li>como tentam deslocar as rela\u00e7\u00f5es de poder no interior do encontro psiqui\u00e1trico.&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>Os autores identificaram defici\u00eancias em todas essas abordagens, no sentido de n\u00e3o estabelecerem pr\u00e1ticas que, de fato, as afastassem das \u201cabordagens e das pr\u00e1ticas convencionais\u201d<\/p>\n\n\n\n<p>Ao final, prop\u00f5em \u201cuma abordagem que busque fundamentar mais firmemente o sofrimento mental no mundo daqueles que o vivenciam, com foco particular nos nichos biopsicossociais dentro dos quais fazemos nossas vidas e no impacto da desvantagem sistem\u00e1tica, da viol\u00eancia estrutural e de outras exposi\u00e7\u00f5es t\u00f3xicas dentro dos espa\u00e7os e lugares que constituem e constrangem muitas vidas cotidianas\u201d<\/p>\n\n\n\n<p>Em concord\u00e2ncia com outras abordagens, refor\u00e7am que uma \u201cpsiquiatria verdadeiramente alternativa requer que os profissionais psiqui\u00e1tricos v\u00e3o al\u00e9m de simplesmente ouvir as vozes dos usu\u00e1rios dos servi\u00e7os\u201d.&nbsp;<\/p>\n\n\n\n<p>Finalmente, sugerem que se \u201coutra psiquiatria\u201d \u00e9 poss\u00edvel, \u201cisso requer uma repagina\u00e7\u00e3o radical do papel e das responsabilidades do psiquiatra medicamente treinado dentro e fora do encontro cl\u00ednico\u201d.<\/p>\n\n\n\n<p>Esse artigo vai bem ao encontro do colocado em artigos recentes na literatura psiqui\u00e1trica que t\u00eam exigido uma nova abordagem para a sa\u00fade mental no Reino Unido, dadas as evid\u00eancias dos resultados muitas vezes ruins ou limitados alcan\u00e7ados pelas abordagens atuais. Uma das principais cr\u00edticas \u00e0 abordagem predominante \u00e9 que ela \u00e9 majoritariamente guiada por um paradigma t\u00e9cnico, o que negligencia o aspecto social da sa\u00fade mental<sup data-fn=\"4e94adb1-9119-423e-a662-9180680a55b0\" class=\"fn\"><a href=\"#4e94adb1-9119-423e-a662-9180680a55b0\" id=\"4e94adb1-9119-423e-a662-9180680a55b0-link\">4<\/a><\/sup>, cuja import\u00e2ncia \u00e9 fortemente apoiada por evid\u00eancias.
